I am an LPN and have done MDS's for 13 years. I complete the nursing parts of the assessment, complete RAP's and the care plan. I ensure all other disciplines have coded accurately, I run the care plan conferences and schedule all assessments. I am considered a "department head" and am in a mangement position. The DON/ADON sign the assessments for completion. I am in Florida, however I did MDS's in Indiana prior to PPS and the DON also signed in this state.

What parts of the MDS process can be done by an LPN? Can they initiate RAP's and Care Plans and have RN co-signature? Currently working the state of Ohio. Is this a by state issue? Please give some ideas on what LPN's do in your facility as far as working in the MDS office.
