Code it as a stage 1. Stage one is not open but the fact that it does not blanch is evidence there is already tissue damage. It should not be interpreted as happening at your facility if a stage one was present on admission.

This heel is mushy, red and purple. It is not open and does not blanche. If I code it as a stage I and it opens later it will look like it occurred in our facility. She was admitted with it that way. Is there any guidelines on how to code a mushy heel? I would appreciate any imput.
