I code a mushy, purple or red heel as a stage one, but make sure I have documentation 
that this heel is looking bad and coulds open at any time. We also put a special boot 
on these tyes of heels and granulex starts right away. In addition we have our 
podiatrist in and he documents on the heel. We have had good survey results with this 
issue r/t using podiatry aggressively.

Subject: coding mushy heels



How would you code a mushy heel in section M.   This heel is mushy, red and purple.  
It is not open and does not blanche.  If I code it as a stage I and it opens later it 
will look like it occurred in our facility.  She was admitted with it that way.  Is 
there any guidelines on how to code a mushy heel?   I would appreciate any imput.
