According to the coding definitions for M1, it would be coded as a Stage 1 if the skin is not broken, unless it is actually a blister - then it would be a Stage 2.

How would you code a mushy heel in section M.   This heel is mushy, red and purple.  It is not open and does not blanche.  If I code it as a stage I and it opens later it will look like it occurred in our facility.  She was admitted with it that way.  Is there any guidelines on how to code a mushy heel?   I would appreciate any imput.    Thank you   Mary Lynn
