Subject: coding mushy heels
Stage I. Intact skin, non-blanchable. Provide COMPLETE pressure relief to
heel, this will prevent from opening. If complete pressure relief is not
maintained, you can bet the heel will break open.
dawn P, RAC-C, CWS
How would you code a mushy heel in section M. This heel is mushy, red
and purple. It is not open and does not blanche. If I code it as a stage I
and it opens later it will look like it occurred in our facility. She was
admitted with it that way. Is there any guidelines on how to code a mushy
heel? I would appreciate any imput. Thank you Mary Lynn
