I don't know if it is different State to State but we do a discharge tracking form return anticipated even if they are not on bed hold.  For us the bed hold policy and D/C tracking form are separate.  For us they can be gone a month and if we think they are coming back we would do a D/C return anticipated and then a reentry tracking when they came back.  If they needed a sig change when they came back we would do that otherwise a Quarterly/Annual what ever was due next.  The RAI manual talks about it on pg 2-24
