You would code it 2/2.
 
You can only code the 8/8 if it never happened at all for the 7 days. 

The self performance and staff support codes are not concerned with the number of times an activity happened (unless you are looking at going from limited to extensive assist or supervision to limited, etc, based on the "3 or more times of more assistance needed" instructions).
 
This example occurs infrequently, but occasionally, with residents on our secure unit.  A couple of them are very intimidated when they go off the unit, and we only take them for special activities or medical appointments, etc. They may only leave the unit once in 7 days, but when they go, they ambulate with only hand-holding guidance.
