the activity doesn't have to occur 3 times to code the MDS.  When you have the majority of 8/8, meaning the activity did not occur, you have to look at what the code would be for when it occured, even if it was only once in the twentyone shifts.   In the scenario you originally questioned, I would code 2/2 on the MDS because the one and only time it occured, that was the level of erformance & support.
