Just complete an annual now. It is late, you cannot change that.  I would write a note explaining that you just realized that an annual was missed and that one was completed as soon as the error was discovered.

Subject: missed annual assessments

What do you do if you completely missed an annual assessment with raps?  The resident is due for a quarterly now and I just noticed that he should have had an annual back in sept.  He had a lot of ins and outs of the hospital and quarterly assessments were done but in Sept  and  Dec and in Sept he should have had an annual assessment to keep on track.  Do I just do it now and do it late.  Its been more than 30 days so I cannot back track correct????

HELP!!!!
