I disagreee with Cindy.  I would not inactivate the quarterly assessments that you already did.  The assessments at the time were done as quarterlys so you cannot "submit a correct one" because a comprehensive annual w/ RAPS wasn't done.  I would leave the completed assessments alone and accept the fact that you missed the annual and complete it ASAP and restart the assessment clock from there.  The annual will be considered late, but you found the error, corrected it, and move on.  If your bosses get upset because you made a mistake, remind them that we are human too.  Maybe as an action plan you can go thru your other charts and verify what assessments are due on all your other residents to verify that you are on track and other MDS's weren't missed.

What do you do if you completely missed an annual assessment with raps?
The resident is due for a quarterly now and I just noticed that he
should have had an annual back in sept. He had a lot of ins and outs of
the hospital and quarterly assessments were done but in Sept and Dec
and in Sept he should have had an annual assessment to keep on track.
Do I just do it now and do it late. Its been more than 30 days so I
cannot back track correct????
