We have a Ford van that has an automatic lift, can hold 7 people, has only one spot to lock down a w/c. Did the disgruntled, terminated employee report that you were transporting people in an unsafe vehicle?? The state came in on a complaint from a terminated employee who called the state, medicare, DSS, employment etc. I work for a wonderful facility for over 3 years. It is owned by a family, 122 beds. Never in the history of this facility have they recieved an immediate Jeopardy "K".  It can't get any worse, for the exception of an "L". What is so sad, even though I am guilty of complaining, overworked etc., I like my job, and care so much for the residents, whether long term or rehab. To work this hard, to have this happen is heartbreaking. It will reflect on the CMS site, and regular people will interpet this as a negative. Thank you for any info.
