If this is what really happened, you need to consult with Public Health. A soft yoke, in an otherwise well cooked egg, is not a hazard.

I know the emotional trauma you are feeling.  Our dietary manager was cooking eggs overeasy for a resident - whites well done - when an immediate jeopardy was cited.  As she was cooking the egg I thought how nice that she would take the time to cook one egg just the way a resident likes it.  Since the yellow was not hard - then the surveyor said the resident was at risk for salmonella and an immediate jeopardy was issued.  Makes you just want to leave the industry at times!
