yes. She was instructed to assist the social worker assistant to sit in the
van for extra protection. This employee was the restorative aid. She would not
clock out when she left the building to go home for a while (lived a block
away from the facility). Stealing time. The sad part of all of this, I
recommended her for the job initally. She had a sparkling 90 day probation
period, then it went down hill. She has been on these transfers via the van
before, w/o complaint. She decided it was unsafe and refused to assist when
she was instructed by our DON. which in turn started all of the hell. We have
a w/c lock down in our van also. But we would run a rope through the w/c
wheels for extra safety, esp turning corners. We only transported two max at a
time. Never in the history of this building >26 years have we had an
accident. The state cited on the "Potential". Like I feel there is a
"Potential" for Bin laden to show up today at our facility. The worse part of
this we will lose our CNA school as well. The administrator, DON an d ADON are
going to informal dispute, but if you are familiar with the guidelines, you
are guilty until you are not. We just want to reduce the scope and severity,
save our CNA school.

It is owned by a family, 122 beds. Never in the history of this facility have
they recieved an immediate Jeopardy "K". It can't get any worse,
for the exception of an "L".
