Can someone give me information, a source etc., where I can find specific information, how your resident is transferred by your facility in a van to go to MD visits and the like. To save time, there is nothing in the "Pink/watermelon" state book and other safety books. What type of van do you have, etc. Ours has the locking system for two w/c's. The state came in on a complaint from a terminated employee who called the state, medicare, DSS, employment etc. I work for a wonderful facility for over 3 years. It is owned by a family, 122 beds. Never in the history of this facility have they recieved an immediate Jeopardy "K".  It can't get any worse, for the exception of an "L". What is so sad, even though I am guilty of complaining, overworked etc., I like my job, and care so much for the residents, whether long term or rehab. To work this hard, to have this happen is heartbreaking. It will reflect on the CMS site, and regular people will interpet this as a negative. Thank you for any info.
