I would not do the Modifications just to get the QI
reports correct. Document the issue so that when the survey comes and asks about
it you have proof that you do have activities and good
participation.
Remember, the QI reports are intended to show
surveyors where to look. They do not prove poor quality.

I have just accessed
our most recent QI reports and we flagged in little or no activity( 95%)! I
investigated and our new Activity Coordinator coded them all incorrectly. He
did not take into account the outside of the formal activity events that the
resident do. Some of these people are very active and involved daily; they
just choose not to go the activity programs ie Bingo etc. Do you think besides
inservicing the Activity person, I should do modifications on all of these

They have to be done BY the
92nd day. You do not have to wait until day 92 to do them. I have
my facility set up on a 12 week rotation. However, 92 days is 13 weeks plus
one day. So, my ARD is 8 days prior to the 92nd day. That allows me some time
to complete the assessments, and allows for a sick day, or day spent chasing
unrelated issues that always seem to pop up.

In our facility
we run a tickler list from the computer and move up the ARD 1 week for the
actual due date to be sure we are in compliance. With an ARD of 3/23
it depends when the last assessment was done if it was done less than 92
days ago from 3/25 than no it does not have to be done. I do agree
that with an ARD of 3/23 by 3/25 it should have been done, but each
facility and MDS Coordinator does things in their own
way.
