First you need to visit with her about the reason she is wanting to leave.  Is she depressed? Is there something from home she is wanting and has not been able to get?  Would she be a canidate for a private room.  If after talking with her, she still wants to go home, visit with the administrator, and don, and her primary physican.  If she is in the nursing home because family is not able to care for her, remind her.  But move carefully and besure you doccument everything that you are doing.  Maybe a different nursing home maybe the answer.  Be sure you do remind her that if she leaves against medical advise, she may not be able to get the help she needs, and her physician may even fire her as a patient.  Be sure to involve everyone in the process, even the ombundsman.

What do you do when a resident is unhappy with a new roommate (but has been unhappy anyway for a long time), and is stating she is leaving?  Her family is here & state they cannot care for her (she is early 60's, right AKA, dialysis, refuses to care for herself at all).... she has said she is calling her lawyer & a cab and she is leaving...  Do we let her go & call DHR or what?  I don't think the MD has been called yet.  I am just here late doing care plans, and the DON has been called back in for this, but we are unsure of what to do.  Is the Ombudsman available now?   Any advice?
