thinned a chart without my knowledge and had thinned out some MDS's into a file
drawer next to the charts. This was a no-no per
surveyor.

These are kept secure in the nurses station report room along with policy and procedure manuals. Just have to be sure to thin MDS older than 15 months and move MDS with room changes. We keep the MDS admit face sheet in plastic page protector on top, then MDS from most recent to oldest.
