When I get a missing resident report I usually find that a Discharge
tracking form has either not been completed or completed but not submitted.

Subject: missing MDS report


The state surveyer who comes in to do the quarterly visits asked me to pull
up the missing MDS report, and that since we have a large facility (180
beds) there should be no more than 12 names on it.  Well, there are 25 on
it.  Dating back to 2001.  She wants them cleared off, but HOW?  The risk
manager and I don't know how.  We can't figure out why most of the people
are on it because our records show the MDSs were submitted/accepted.  Can
anyone help me?
