What I find is usually the case, besides an actual missed MDS, is that the discharge tracking form was either transmitted at the same time as the final assessment transmitted or the discharge tracking form is transmitted before the final assessment is transmitted.  The usual reason for this is the required PPS assessments for payment.  If a resident is discharged within the first 8 days, the facility completes a 5-day PPS assessment.  The facility has 14 days from the ARD to complete the assessment.  So, very often, since we must complete the discharge tracking form within 7 days of discharge, the discharge tracking form is ready to go before the assessment.  The state data base receives the discharge tracking form and then when the 5-day assessment is received at the state, the data base believes that the resident is still an active resident because it has received a valid assessment.  The instructions for transmission recommend not sending a discharge tracking form before the final assessment has been transmitted for each resident to prevent names appearing incorrectly on the missing assessment report.  If you have kept records of your tranmission batches, you could check to see if this , indeed, was the cause of your residents appearing on the report.  If you can show this to the surveyor, it should allay their concerns. 
This is very hard to keep track of, especially in a large facility.  I try to keep my Medicare list by my transmission computer and double check to make sure that I have sent the final assessment for those Medicare residents who have been discharged.

Subject: missing MDS report

The state surveyer who comes in to do the quarterly visits asked me to pull up the missing MDS report, and that since we have a large facility (180 beds) there should be no more than 12 names on it.  Well, there are 25 on it.  Dating back to 2001.  She wants them cleared off, but HOW?  The risk manager and I don't know how.  We can't figure out why most of the people are on it because our records show the MDSs were submitted/accepted.  Can anyone help me?
