That is my opinion as well.  The VB2 is the completion date of the RAPs.  The VB4 is the date that the care plan must be in place and the key is to have the care plans interdisciplinary.  You have 7 days from the VB2 before the VB4 must be signed but no later than day 21.  You actually don't have 21 days for care plans if you do the comprehansive MDS with a 5 day PPS MDS - only 7 days from VB2.  (See RAI page 1-25 & 2-21).  With that in mind, I often do the admit MDS with the 5 day PPS and as long as the care plans are complete and interdisciplinary within 7 days of VB2, I don't set my actual meeting until a week or so after that.  We meet as a team every day and discuss care issues that may need to be added to care plans, but as far as an actual meeting with all disciplines and resident and family all together at one time, I wait until we get a good picture of how the person is progressing in rehab and their potential for discharge before we set a meeting.  (And yes, it's usually week 3 of their stay).   If someone goes out to the hosp after the VB4 but before our "formal care conf meeting", then we'd reschedule after their return.---------------Mike

I am having the issue of everything being done and in place , then the resident gets admitted to the hospital before the actual "review meeting", and I"m wondering can we have that "meeting" later, when they return (no sig change)?
