At some point you are probably going to have to migrate your client database to another program.

Unfortunately, databases grow continuously and live longer usable lifespans than DB programs so your problem is one of the chief concerns and problems for professionals. You should check on the db export capabilities of your current program and also of it's successor. Maybe the new version can export into a generic file type that can be imported by many other programs. Or maybe you can find a new product that can translate your DB into theirs.

I would think that your Hippocratic obligations of privacy require that the client database that you use is encrypted and secured by a password.

I use Palm Organizer myself but it seems that it seems to be nearing the end of its popularity and lifespan so I'm not sure it would be a wise choice.

You should poll your peers for a list of Hippocratic acceptable programs they use. Pick the most current and most popular. Spend money if need be. It won't be cheap if you have to hire a DB specialist to port a dead ended db to another program.

resources. Save your file and close Organizer". Yes, I still use Lotus
Organizer 5 for my appointments".

Well... I am here happy with my brand new computer and then...the same
message! Almost fell from my chair... What internal resorces are
insufficient now? My God... this new plus computer also?...
