Just set up using a default configuration, and put your domain in esmtpacceptmailfor (and run makeacceptmailfor). Don't set up your domain/hostname as your backup server's domain/hostname. It should have distinct hostname.
As long as your domain's DNS records are sane, everything should work correctly. When Courier sees a high priority MX record for a hostname that it recognizes as its own hostname, mail gets automatically forwarded to the lower priority MXs.
Your server should have access to stable DNS, in order for this to work reliably. Alternatively, you can also explicitly set the primary server in esmtproutes, in lieu of using a DNS-based configuration.
Note that your backup MX has no access to the list of mailboxes on your primary MX. All mail for any address @ your domain will be accepted, and the nonexistent recipients will be bounce from the backup MX. This will generate backscatter, which should not really be much of an issue for a backup MX.
