I could. But no-one has the knowledge or time to setup a webserver and create form processing HTML/scripts. On the other hand, everyone that's part of our neighborhood knows PDF and understands e-mail. It's "KISS" technology, with heavy emphasis on the "simple, stupid." So downloading (or e-mailing) a PDF, filling it out, and e-mailing it back is quite attractive for us. Our current idea is to do the survey using a Word document, but nobody likes that idea because 1) the current mindset is that "Word attachments transmit viruses", 2) we don't know of a way to prevent people from only typing in the fields and not messing up the rest of the doc, and 3) not everyone has Word. We could do the same using a text file, but though a PDF might be a better solution.

Actually, currently we are mailing (as in, postal, aka snail mail) out printed version of the survey, asking people to write in their response (as in, with a pen or pencil), and then mail them back or drop them off.
