Currently we use Yahoo!Groups. The price is right and it does more
than everything we need, except this one survey. Also, the group has
a web site on the City's server. Don't know too much about it other
than the only way to access it is via FTP.
Or use a survey builder site already on the net. I know they exist but
dont know any names.
