Playing devil's advocate, why does this need to be a database
application? Yes, it would be cool and snazzy to have all these
gadgets, but maybe the least headache and portable solution is to
print out the PDF form at a local Kinko's and fill it in by hand.
It's low-tech, requires minimal training, no batteries, easily copied.
