See the prompt? I'm thinking you should never see that. Most ISPs now use PAP, or in rare cases CHAP, to authenticate. The PAP/CHAP protocol calls for the peer (your computer) to begin sending "request to authenticate" packets as soon as the modem connects.
I have seen ISPs that, having not received the PAP/CHAP auth requests in a timely manner, will fall back to a menu such as the one you're seeing. There should be +pap and +chap somewhere in your PPP options file.
