Have you tried the profiled denoise with either a simple reduction in strength 
or applied with a brightness ramp through the parametric stuff Halgeir?    I 
have one of the older Pentax cameras that (unlike the current range)  has a 
fair bit of noise at high ISO and like you find the profiled denoise a  bit 
much at high ISO although brilliant at normal values – just a bit of blending 
works a treat with it though and I generally only go to NLM or bilateral NR for 
special cases.

While we are on the subject of denoise - if I could choose something for my 
Xmas list  it would be for dt to keep track of how much each pixel (or area) of 
the screen has been boosted in brightness by the various modules and then offer 
denoise (or a mask) based on the effective ISO we have raised those  pixels to. 
 I don’t know if that would be practical  but it would be really, really nice 
and save a lot of faff manually finding  denoise selection parameters  to match 
the effects we have used.  What we already have is fantastic though so I’m just 
being greedy ☺

Denoise (profile) removes to much details for my taste so I have to use denoise 
(non local means) and the equalizer

To follow up the case further. The problem is on raw images from noisy cameras 
like Canon 7D on high iso levels.
Give me a couple of days and I will dig in my archive and find those 
problematic raw images, if I still have those shots.

Basically the problem was in images with iso 3200 and greater.

On other camera systemes with a different sensor like Pentax, Sony og Nikon 
this is a non existing problem.
Canon sensor don't handle noise very well and the raw files is a pain to 
process i Dt.

threshold if it doesn't fit your needs. Apart from that, you only have to use 
the 'Denoise profiled' (which as something like 2 useful sliders, and often

from LR. I don't realy see the point in introducing yet another noise reduction

but it don't always work.

I hope that some of you have experience with other raw processor. But
this is the situation.
Lr have none of these problems and do a perfectly job with only 4
sliders. Rawtherapee solves
this by 6 sliders in noise reduction and the white dots are removed by
Impulse Noise Reduction
filter. Both raw processor do a perfect job.

The most easy solution in my opinion is to add a Impulse Noise Reduction
alike tool.
