I have much less of a problem in this regard than I had a year or so 
ago; the developers (I thought) had in the main fixed the main issues in 
this problem but it may also be that I am now more cautious in keeping 
highlights under control at the time of the exposure.

While there are a number of ways to fix the problem after the fact .. 
all of those routes are a nuisance. Best to start looking at the way 
that you are metering the scene.
