Sorry it's taken me this long to get back to you on this issue.

I have installed a WinNT 4.0 system under Virtual PC and have done some
experimenting with it. However, without actually having your printer
connected I can only do so much testing here. So you'll need to try some
things on your end.

First of all, we need to determine whether Windows NT itself is able to
print correctly to the printer's full width using the driver you have
installed.

I would start by creating a custom form size in your Windows print
server that reflects your exact paper size. Click Start | Settings |
Printers. Then click on the File menu, and then the Server Properties
option. On the Forms tab you should see a long list of paper sizes. We
want to add your custom size to this list. Check the 'Create a New Form'
box and then type in a name for it where it says 'Form Description
for:'. Perhaps something like 'Custom 345mm x 215 mm'. Now select Metric
for the units and specify 34.5cm as the width and 21.5cm as the height.
You can also specify the minimum margins here if you want to. Click on
the 'Save Form' button to save your new custom form size to the list and
then click on OK.

Now try creating a document with a Windows word processor (or even
WordPad) using your selected printer and that new custom page size and
some appropriate margin settings, with text that runs all the way
across. See if it can print all the way across the full width.

If you can print full width with the word processor, that tells us that
the driver should be ok and I'll look further into the DPSpool code to
see if I can track down what's going wrong with DPSpool. But if even the
word processor can't print to the full width, that tells us that it's a
driver issue.
