The per-user and per-domain settings are done based on the "actual
recipient", rather than the "intended recipient". That means that any
translations that IMail does before Declude is called will be taken into
effect, such as domain aliases. So for the per-user and per-domain
settings, you should use the actual recipient's domain (in this case,
mail.webhost.hm-software.com).

There was a minor change made for v1.28, that should only occur with a new
config option, so I'll have to check to make sure that is working properly
-- I thought that the "To:" would show the intended recipient, not the
actual recipient.

Yes. Declude does use the "intended recipient" for those variables. That
means that if you send a virus to "[EMAIL PROTECTED]", you won't get a
bounce message back saying that it was to "[EMAIL PROTECTED]" (to keep
the actual addresses hidden from the user).
