I think we agree on everything but some semantics.  When I hear "user
error", I don't scapegoat the user, instead I focus on how that can be
prevented in the system design.  What needs to happen is a change in
response on the part of designers who think that "user error" is really the
fault of the users and if that works in your organization by simply
restating "error" as "unexpected incident" then do it!
