In my case I know which device it is, it's the router :) It's having these issues with almost all of my devices (excluding, interestingly, the Squeezeboxes, but the server might be affected, too), I've seen similar issues with other people and other devices (including Squeezeboxes). It might be a chipset compatibility issue or something.
Anyway, short term it often helps to restart the router. At least in my case it comes back (longer term), though so I use another router. It's definitely not the only issue at hand, though. I think there might also be a software issue on the server side or a protocol issue or something but obviously sometimes the internal position within the server gets out of sync with reality causing trouble.
