Voiceover is quite advanced and good with the mouse too. unlike jaws where for instance, you use the jaws cursor and get "blank "blank "blank" all over the place, you can acutally use the mouse with osx and voiceover will speak where you are.
To answer some of your questions though, if you are a windows user, you'll want to set it up at least at first the way you set up windows. I personally use tones where I can and as few bars as possible. as far as a search engine is concerned, I just type it in.
