Hi :)
First and last names only.  Sometimes only the initial for the first name.  
Seldom get asked for middle initial let alone middle name.  Guess there are 
fewer of us in the UK.  Hmmm, actually rarely get asked for name at all.  
Usually Date-of-Birth  or National Insurance Number is used to identify 
people.  (I'm not a number.  I'm a free man.)
