With XMLUI, an administrator is not able to see tasks that s/he has not submitted or is in the approval group for. I suggest looking at the membership of the step 3 approval group for that collection and contacting someone in that group. In managing our repository, we use a "Notify" group that includes select administrators. We add this Notify group to all collections that we aren't responsible for approving. That way, when this type of event occurs, we can access to the item, as long as someone hasn't already claimed it from the task pool.
