Can you make it trip relatively at will with the tighter tolerance? Maybe set back to .003" and run Halscope with it monitoring PPMC.0.encoder.xx.delta and pid.xx.error and maybe also the motion enable signal and setting the trigger on that one turning off. One thing that is very unlikely to show up on halmeter is a single sample of bad position. If there is electrical noise or anything of that nature, it could cause momentary misreadings of the encoder count. A bad ground on a spindle drive, or the machine itself, might introduce noise that could corrupt the computer-PPMC communication.
Since there are suddenly problems on TWO machines, I'm thinking about electrical disturbances on plant's mains having something to do with it.
