It seems I can't SELECT an app from the list because the list is empty and 
the Add button is disabled:

When I try to use the "personal application launcher" it doesn't seem to 
work. Do I need to fill in every field?
As an example, I tried filling in for xterm as follows:
Name: xterm
Comment: Blah
Application: /usr/bin/xterm
Icon: Selected a jpg
Run in Terminal (ticked)
Show in menus (ticked)

When I press OK the window disappears and nothing seems to have happened?
