What does the console port say when you restart the blade? Could be POST failure (Power On Self Test). Does the blade work on its own when no other blades are inserted in the chassis? I have seen this if modules have incompatible DIP-switch settings, that's why I suggest trying the blade by itself. Does the neighbouring blade's console say anything when you insert the 'faulty' blade?
