I find that if there is a bright light source in the frame it will throw off  the 
meter.  I photgraph a lot of outdoor weddings] and finf that the meter can be fooled 
with the white dress and black suits.  Generally when I am taking other portraits I 
use the hand held meter to read the light falling on the subject.  I seem to get 
better more consistent exposures.  

Sometimes if the background is bright, I take a readin from the camera pointed at the 
background and let the fill flash bring up the light on the subject.  If I am not 
careful, the bright background will throw off the flash sensing and underexpose the 
subject.
