You've left out two critical piece of information:
which EOS body do you have, and what do you do with
it? My EOS 3 has a remarkably accurate meter, and I
never use an external meter any more (except for a
flash meter, occasionally). But if I were doing a lot
of studio work that might change.
My Elan IIe handles back lighting quite poorly. I
might seriously consider a spot meter if that were my
primary camera (but given the price of good spot
meters, I'd probably just sell the Elan IIe and buy a
3, instead.
As for Macro, I think an external meter would be a
royal PITA. Juggling the meter and camera when both
need to be so close to the subject would be a
nuisance. And if you're using extension tubes, and
even some macro lenses, you'd have to manually
calculate the light loss and adjust the exposure
accordingly.
Use a body with a good metering system, and leave the
handheld meter in the store. MHO,
