I have two LowePro models that more or less fit the bill:
Pro Mag II AW: big enough as long as you reverse the hood of the 100-400,
with room to spare for smaller extra lenses or even another big one:
the 100-400 also fits on the side with body plus another lens attached
on top. About as convenient as a shoulder bag can be, the belt helps
to put part of the weight on my hips and keep the thing in place
when I swing it behind myself and kneel to look something low.

Off Trail II: main compartment just big enough to hold EOS 3 + 100-400
with hood reversed, removable side compartments hold the 28-135 with
hood removed and put in sideways, ditto with 20-35/3.5-4.5, the
100/2.8 USM macro fits even with hood on right. The best thing I've found
for keeping the 100-400 easily accessible in hard terrain, I've
even used it on horseback (just tighten the belt real tight).
The 100-400 doesn't really fit in the side tubes however
(well it can be squeezed there if you remove the tripod collar,
I've done that a lot too but it's not exactly convenient).
I don't know if the 20/2.8 would fit, probably also only
with hood detached.
