Marc, How far were you from your subject? What f/stop did you use? When using the open-flash technique you often have to flash your subject several times and consider the distance flash-to-subject and the f/stop for a good exposure. The 380EX has a GN of 38m at 105 mm zoom and it *does* fire full power when you push the pilot button, but if I can recall correctly, when you use it off-camera, it resets to 50mm coverage, what brings your output down. You also have to consider that outside, there're no elements to bounce light back on the subject (like walls and ceilings indoors) and you need more power to achieve some results; firing many times is again an option.
How many times? that's a question you need to answer by trial and error... don't be give up after the first try.
