Interesting, of the three lenses I have (EF20-35, EF28-135 and EF100-400)
and the A2 I had and now EOS-3 bodies I've used all three lenses on, I've
ONLY seen it very occasionally on the EF100-400 mounted on EOS-3, this has
been going on after I changed to the EOS-3 about 4 years ago.

A quick (partial) dismount and remount of the lens cures this problem,
every time.  I don't think turning the camera off and back on does the
trick though in my case.
