I would also try reformatting the card while in a PC card reader, with
verify/test after re-format. Then do a disk error check on it, then, if
it reports OK, then reformat it while in the camera and test the image
file creation again under similar circumstances.
The Windows FAT-type file system technology is not the most robust, in
my experience.
