It certainly appears that one of the worst predictions of the report
is being thoroughly borne out so far, as far as response - we've seen
personnel purges at NASA and we've seen congresscritters squealing that
they haven't gone far enough, but nowhere in this frenzy for "goat meat"
do we see any effort to reform the organizational parameters that led two
separate sets of good people, 17 years apart, to make similar sets of
flawed decisions.
