Or just the microswitch or pot position needs adjusting. The trouble could likely be caused by the controller is seeing some throttle input before the microswitch has managed to turn on the main contactor. As a result the controller goes into high pedal disable mode. This mode is intended to keep the car from taking off if the controller is turned on while the throttle input is on.
