I had simlar problems, but much higher Averaged RPC Latency time
(climbing), very little in the queues, disk performing fine.

Do you have any processes with unusually high cpu %

Our problem turned out to be GFI Mail Security, getting stuck
decompressing archive attachments and creating large files in the GFI
proc folders. (several 4gb files).
Either it was a problem decompressing, or GFI was not recognising mail
bombs and decompressing them correctly.

I found the MS Exchange Performance tuning document quite interesting.
