How are your Recipient Policies configured? If you have them defined correctly, try changing the priority of the recipient policies and then change them back. Recipient policies can be a little tricky sometimes and not do what you expect them to.

So far, it appears that it is only deleting the secondary SMTP address...but it's completely random from what we can tell. There is no time period in particular that it happens, nor does it make a distinction between new or old mailboxes. We might have three addresses disappear in a week and then it might happen again next month and then it might not happen again for three months. I have heard of mailboxes being deleted by other admins, e-mail disappearing from mailboxes, users disappearing from the GAL, but never heard of SMTP addresses disappearing from random mailboxes.
