What does your .pinerc permissions show?

Mine are: ls -l .pinerc

'user' -rw-rw-r-- 1 user user 13933 Dec 1 06:15 .pinerc
'root' -rw-rw-r-- 1 root root 13683 Dec 1 06:15 /root/.pinerc

This is stock install of LM 7.2 with 'normal' security settings
I am also using Postfix instead of Sendmail which is the stock way
my mail setup installed itself as.
