Nityananda, Guru to Muktananda sadly left his body early declaring that there were none ( and Muktananda is one of his disciples), and this was not long ago.
Even in this case, it was probably not an ouch as the methodology was reasonable. The real ouch is where there are none enlightened after a long presence - then in looking at the methodology, it may be quite obvious why this has happened.
